Secondary Transition Planning for Students with Disabilities

The Ohio Department of Education’s Secondary Transition Planning for Students with Disabilities webpage holds updates and resources for educators and parents who want to learn about transition in Ohio. The information includes resource, information on transition planning, and rules and regulations. Resource Links for Secondary Transition include Understanding the Summary of Performance, the Ohio Longitudinal Transition Study, and the National Technical Assistance Center on Transition.

 

 Ohio Transition Support Partnership for Students Age 14

Available from the Ohio Department of Education’s secondary transition webpage is a link to Ohio Transition Support Partnership information, a partnership of the Ohio Department of Education and Opportunities for Ohioans with Disabilities to increase the availability of vocational rehabilitation services for students with disabilities beginning at age 14. The Ohio Transition Support Partnership is an innovative collaboration will change the nature and impact of transition services for students with disabilities in Ohio. The new model will help students with disabilities get a head start on becoming job ready and better prepared to enter the workforce with the skills necessary to be successful. As a result of this partnership, 26 dedicated vocational rehabilitation counselors and 13 caseload assistants from Opportunities for Ohioans with Disabilities will work with transition-age youth throughout the state.

Career Connections

Career Connections is a joint initiative among the Governor’s Office of Workforce Transformation, Ohio Board of Regents, OhioMeansJobs and Ohio Department of Education. Career Connections began in 2012 by providing a framework by which students develop a vision and realistic plan for their futures – during K-12 and beyond. Career Connections provides decision-making materials, information and tools that easily fit into classroom instruction at all grades levels.

 

Office for Exceptional Children’s LiveBinder

The Ohio Department of Education’s Office for Exceptional Children constructed the Resources for Essential IEP, Essential ETR, and Post-Secondary Transition LiveBinder filled with resources and information. The post-secondary transition resources include Post-Secondary Goals, AATA and Surveys, Services and Activities, the Summary of Performance, and additional teacher resources.
